Any individual hired to work at an EPS event must first complete and submit this form each calendar year.
This form must be filled out in its entirety. Incomplete forms will not be accepted.
Initial each section that you agree to the code of conduct and fill out NDA & W9. *Sign all and submit.
You must agree to all of the terms of this document to be eligible for hire.
To be paid in a timely manner, this form needs to be filled out and submitted at least 72 hours before the event.
ONCE YOU ARE IN THE SYSTEM, YOU WILL NOT HAVE TO DO THIS AGAIN UNTIL NEXT YEAR.
